Renewal fee of six times the monthly fee applies if membership is discontinued at any time. The monthly fee amount increases each January 1. Memberships are non-refundable and non-transferable. Each member is responsible for scheduling and completing their annual appointments. Appointments can be scheduled by phone, text, or online at www.oremdmd.com.
Appointment cancellation fees still apply. Membership Exclusion: Unfortunately, in the event of active periodontal disease, routine cleanings and exams cannot be performed until the necessary restorative perio procedures are completed. In this case, the 10% discount will be applied toward the prescribed periodontal procedures, and any membership plan fees will be credited to your account.
